How much ease should I add?

There is no figure agreed across the industry. If the product notes give a recommended ease, use it. Otherwise try a small amount for stretchy garments and a generous amount for thick ones such as padded coats, then compare the two results.

Why does it not just tell me S, M or L?

A medium from one brand is a different chest girth from a medium in another. With no industry wide chart to work from, comparing against the actual measurements of the product you are considering is more accurate.

What goes wrong if the back length is short?

A garment that is too short pulls across the back and restricts movement, while one that is too long is easily soiled during elimination. Among the sizes whose chest girth fits, pick the one whose back length is not short.
